SMS Communications Consent

Exciting changes and improvements have recently occured within the Medical Software that we use at Glebe Hill Family Practice.

Our software now has the capacity to allow us to send SMS reminders to patients about Clinical Information, Results, and Practice Information, rather than just about Appointment Reminders and Recalls.

You might start to receive SMS messages from GHFP titled Best Health.

We would like to take this opportunity to check that we have the correct mobile phone number for you and your family members, and that you consent to receive communication in this manner.

If you have any questions, please phone 6169 0000 to speak to one of our friendly Team members.

If you don't consent to SMS contact the alternatives are for us to contact you by phone or mail when there is something important we need to notify you of.

There are 4 types of SMS communications that we require your consent for:

  1. Appointment Reminders

  2. Clinical Reminders (eg. Things that are due, such as Cervical Screening Test, Diabetes Review, or Blood Pressure Check)

  3. Clinical Communication (eg. Short message to notify you that your test results were normal, or that your Dr has requested to see you to discuss your test results)

  4. Health Awareness (eg. Important practice information to patients, such as availability of flu vaccines, changes to services, changes to opening hours) 

The previous New Patient Consent Form you have previously completed at GHFP is not specific enough for our new SMS communication system.

Please print and fill in and sign the Communications Consent Form - Existing Patients and return it to Glebe Hill Family Practice as soon as possible. You could return this to us the next time you come by the surgery, by email, or by mail. We will store the signed consent form in your electronic patient record.

We apologise for the inconvenience and thank you for your help with this. We are confident that the improved function of the new system is worth the inconvenience of the extra paperwork!
